What is a Medical Home?
A medical home is not a building, house, or hospital, but rather an approach to providing comprehensive primary care. A medical home is defined as primary care that is accessible, continuous, comprehensive, family centered, coordinated, compassionate, and culturally effective.
In a family-centered medical home, a pediatric care team works in partnership with a cild and a child’s family to assure that all of the medical and non-medical needs of the patient are met. Through this partnership, the pediatric care team can help the family/patient access, coordinate, and understand specialty care, educational services, out-of-home care, family support, and other public and private community services that are important to the overall health of the child and family.
NDAAP Medical Home Liaison
Kora Dockter serves as the Medical Home Liaison for the North Dakota chapter. This position is made possible by a grant through ND Children’s Special Health Services. Kora assists physician practices in implementing medical home through determining reimbursement options, writing grants to support activities such as care coordination and education, and supports our membership in grant administration responsibilities related to medical home. For instance, she will be available to support you by writing grants on your behalf, and provide technical assistance in writing grant reports.
